  • Patient / PWLE  name and discharge or housing status
  • Current location (hospital, home, care setting)
  • Destination address (home, nursing home, supported living)
  • Required services (see our packages or request custom support)
  • Preferred discharge date

Our team will review the referral  and contact you to:  


  • Discuss service needs  
  • Clarify logistics and access 
  • Confirm cost 
  • Schedule delivery and setup  
  • We coordinate directly with discharge teams, families, and care homes to ensure everything is aligned. 

Step 3: One Visit, Full Setup

On the scheduled day, our team delivers and installs all requested items in one coordinated visit:

  • Home cleaned or cleared
  • Chosen service and packs delivered and installed
  • Safety equipment fitted (key safe, medicine cabinet)
  • Food parcel 

We leave the new environment ready for safe, dignified arrival.
